The Diabetes program is currently for patients who have been diagnosed with Type 2 Diabetes. Our team is working together with the Halton Healthcare Services’ Diabetes Program to provide patients with comprehensive diabetes care. Patients will have the opportunity to attend individual appointments or group education sessions to assist in managing their diabetes. The program will provide patients with the knowledge and skills for optimal diabetes management including information on nutrition, glucometer use and medications. Patients are able to ask questions, be involved in decision-making around their health care and take responsibility for managing their own health.
The goal of the Diabetes program is to empower patients with the knowledge and skills to optimally manage their diabetes on their own and improve their quality of life.
A referral must be made by your family physician to join the Diabetes program.
Our Family Health Team will provide you with comprehensive COPD care where you will receive education by our health care team to assist in managing your COPD. Patients are encouraged to ask questions, be involved in decision-making around their health care and take responsibility for managing their own health. Patients will have the opportunity to meet with specialized members of our health care team to discuss smoking cessation, medications and other quality of life issues that may arise that affect your ability to manage your COPD.
The goal of the COPD program is to empower patients with the knowledge to optimally manage their COPD on their own and improve their quality of life.
A referral must be made by your family physician to join the COPD program.
The goal of the Asthma program is to provide efficient and accessible services to patients to assist in the treatment and management of Asthma. Our team is working together with the Halton Healthcare Services’ Asthma program to provide comprehensive Asthma care. Patients will have the opportunity to learn what asthma is, how the lungs function, correct inhaler technique, triggers, warning signs and symptoms, develop an asthma action plan and set goals for optimal asthma management.
This program will empower patients with the knowledge and skills to better manage their asthma and improve their quality of life.
The Heart Function program is for patients diagnosed with Congestive Heart Failure (CHF). The goal of this program is to provide you with comprehensive education about heart function and to support you in taking charge of your health. Our Nurse Practitioner and other members of our healthcare team will meet with you to provide education about heart failure and any other issues that may be affecting the management of your heart failure. Information about nutrition including salt and fluid restrictions, medications and symptom management will be provided. Patients will have the opportunity to ask questions, be involved in decision-making around their health care and take responsibility for managing their own health. Patients will be followed throughout the program to monitor progress.
The goal of the program is to empower patients with the knowledge and capacity to manage their condition and improve their quality of life.
A referral must be made by your family physician to join the Heart Function program.
Our Psychometrist and Mental Health Workers collaborate to help individuals and families with mental health related issues such as:
The Mental Health Team uses a variety of treatment modalities in their consultations including Cognitive Behavioural Therapy (CBT) and Interpersonal Therapy (IPT).
Mental health services are provided for children, teens, adults and seniors. Our services are offered to patients that are unable to afford private assessment or treatment services and do not have access to an employee assistance program or other insurance related options.
Our physicians may make a referral to the mental health team when they recognize high risk behaviours in their patients, or when a patient may be experiencing difficulty coping with a life event. Patients may also be referred for an assessment for learning disabilities, ADHD or personality screening. Brief short-term counseling is also offered, ranging from 1-6 sessions with a member of our mental health team. Upon completion of the sessions, patients may be referred to a community agency for long-term therapy.
Our Mental Health Team also facilitates group education programs.

Our Speech-Language Pathologist (SLP) works with clients and their families to improve communication skills. Our SLP provides services for infants, toddlers, preschoolers, school-aged children, adolescents, adults and seniors with any of the following:
Any of the above issues could be related to a developmental issue, or due to a traumatic brain injury, stroke, dementia, or degenerative disease. We know that early intervention can make all the difference.
Clients are seen for assessment and based on the findings a treatment plan is developed by the family, client and SLP on a weekly or bi-weekly treatment schedule. Our SLP services can also complement services clients are receiving in the school, daycare or day hospital setting.
Through lifestyle modifications, we can help prevent chronic disease and improve the management of existing conditions to enhance quality of life. Through individual consultations and group education programs we can provide patients with the knowledge and skills to better manage and take control of your health. Through diet, exercise, medication and managing mental health, we can help you make the changes you need for optimal health and wellness.

Our Dietitian provides nutrition counseling and education to both individuals and groups. You and your family will learn to make wise eating choices that promote good health at any stage of life.
Our Dietitian also provides specialized nutrition therapy to help you manage nutrition related illness like prediabetes, high cholesterol and high blood pressure.

Our health care team can assist you in butting out! The thought of quitting can be quite overwhelming for some people. Our team can guide you through the quitting process by providing education and support around your readiness to quit and teaching you the skills and techniques that will assist you in making the change to becoming smoke free. Depending on your readiness to change, education will focus on barriers to quitting, behavioural strategies to cope with triggers, pharmacotherapy and Nicotine Replacement Therapy options, relapse prevention and maintaining a smoke free status. Counseling is goal directed and patient centered to empower and build capacity to quit with confidence.
The goal of the Cognitive Assessment Program is to gather the necessary information and relevant cognitive test scores to aid the physician in a potential diagnosis of Dementia. Patients are referred to the program by their family physician or other healthcare provider to screen patients that are having memory concerns. The program is open to patients that are medically stable and over the age of 65 for screening purposes only. The patient meets with a Registered Nurse for approximately 1 hr and completes several universal screening tools for cognitive decline. Our program also provides access to appropriate supports and resources for patients and their families in the community. The patient may also see some OR all of the Allied Healthcare Team, which consists of a Pharmacist, Health Educator, Mental Health Workers, Dietitian and Nurse Practitioner. Upon completion of the program, the family physician will be given the collected information, test results and recommendations. The family physician will use this report to formulate a diagnosis.
