The Halton Hills Family Health Team is one of many Family Health Teams in the province of Ontario. A Family Health Team brings together family physicians and allied health professionals to coordinate the highest possible quality of primary health care for you – the patient. Designed to give improved access for patients to physicians with support from allied health care professionals, our Family Health Team consists of doctors, nurses, nurse practitioners, dietitian, mental health workers, psychometrist, pharmacist, health educator and speech language pathologist, working collaboratively, utilizing their experience and skills to provide your care, when you need it, as close to home as possible.
The Halton Hills Family Health Team is also here to assist our patients in navigating the healthcare system and provide you with the necessary tools and resources to improve and maintain your health.
