The Halton Hills Family Health Team wants to ensure we are consistently providing quality healthcare to our patients. Over the past year, our our Quality Improvement Team has been participating in the Ministry of Health and Long-Term Care, Quality Improvement and Innovation Partnership (QIIP) initiative. Our team is currently focusing on quality improvement in the areas of chronic disease management, preventative care and office practices.
